What is the HTML code to create a feedback form?

I have created my blog www.ultimate-proxy.blogspot.com which gives the user a list of free proxy servers to help them have an anonymous browsing. So …… Can anyone tell me the HTML code to create a feedback form with a submit button. Most important is the action part. Where input comments to be saved? Please give me the codes and give me some instructions how codes work!

Unless you want to go through the trouble of making a cgi bin, I suggest that the easiest way but a little boring to mail comments to yourself then paste manually on the server. Email@email.com change your own address Changing the text area rows and cols (width and length of your comment box, the length for "next line" (the enter key) and the width of the space) of custom text to the "comment" to send the comment to you.
